Care Coordination

Does your family need support with the demands of care?

Drazen Law Group, LLC can help. We are one of the few law firms in Connecticut to offer customized care planning and coordination services, which is an important component of Life Care Planning. Whether your loved one is an elderly relative, a disabled dependent or another cherished member of your family, we can ease the burdens of care.

What does a care coordinator do?

A care coordinator helps your family develop, implement, and monitor care arrangements for elderly or disabled loved ones. A care coordinator is involved in all discussions about finding and paying for care, helping you weigh the financial implications of each care decision.  Services include:

  • Conducting assessments to identify needs and develop appropriate care plans
  • Helping families find resources and community services
  • Monitoring in-home health or companion services
  • Acting as a liaison between the elderly or disabled loved one and family members living at a distance. In addition to monitoring the loved one’s situation, our firm provides a local presence who has detailed knowledge about local service providers and community resources.
  • Transitioning the elder to a new living situation including a retirement community, assisted living or a skilled nursing facility
  • Reviewing care issues and referrals to appropriate community resources as necessary
  • Providing independent advocacy to the elder and their family concerning health care products and services
  • Providing input on the quality and availability of resources within the community

Care coordination gives you a new way to honor your commitment to care for loved ones while minimizing the burdens and burnout that are all too common among family caregivers. Care coordination can restore your peace of mind.

Frequently-Asked Questions

Find answers to common questions about care coordination.

What is a care coordinator?
A care coordinator is a health and human services professional trained to understand the issues related to disability and aging.

How can a care coordinator help me?
Your care coordinator can help you navigate the tricky maze of health care, long-term care and public benefits, while being sensitive to the complicated family dynamics that often occur during these stressful times.

Our parents want to live at home but it doesn’t seem safe. How can you help us?
It starts with an evaluation of your parents’ home as well as their health status. That assessment tells us whether it’s possible to modify their home to make it safe. It also will help us determine whether home care services might be appropriate and what options exist to pay for that care.

We were told that our mother may need nursing home care. How can we be sure it’s right for her?
We deal with many clients who are confronted with the same issue. Many elderly people have heard horror stories of nursing homes, and thus are biased against them. Our goal is to always keep people in the least restrictive environment possible while maintaining their safety.  But we also weigh the potential benefits the nursing home may provide, such as socialization and safety. We work with many nursing homes, so we can suggest which ones may be better options for your Mom, but in the end the choice is always yours.

Is a nursing home the only option if a parent can’t stay home?
There are many other options available. All provide different levels of care. Payment options also vary. Some facilities are private pay, meaning you pay out of pocket. Others accept patients on Medicaid or other public benefit programs. There are many options and many variables, which makes it very difficult for the average person to know what to do next. This is where Drazen Law Group, LLC can be of great assistance. We understand all the options and can help you make the best decision for your loved ones.
